Pulmonary sarcoidosis: case reportA 43-year-old man developed pulmonary sarcoidosis during

antiviral treatment with peginterferon-α-2a and ribavirin[routes not stated] for chronic hepatitis C infection.

The man began receiving peginterferon-α-2a 180 µg/weekand ribavirin 1000 mg/day, in 2009. In the fifth month oftreatment, he reported fever, chills, sweating, chest pain,fatigue and headache. He had a fever (38°C), submandibularadenopathy, and sonorous and subcrepitant rales in his rightlung. He had severe thrombocytopenia but no radiologicalpulmonary changes.

Antivirals were withdrawn for 7 days and the man receivedantibacterials. Radiology after 7 days of antibacterial therapyshowed a focus of right infrahilar condensation. Hisrespiratory symptoms faded within 14 days and histhrombocytopenia improved. Radiology at 21 days showed adecrease in size of the inflammatory focus. Over the remaining7 months of antiviral therapy, his platelet and leukocyte countsfluctuated. He received symptomatic treatment forintermittent fever, dry cough and fatigue. After 48 weeks ofantiviral therapy, he again reported cough with purulentexpectoration and progressive dyspnoea. Radiology showedbilateral hilar lymphadenopathy and tests showed significantpulmonary and cardiac changes. Phenotypic analysis of abronchoalveolar lavage sample showed findings consistentwith sarcoidosis. He began receiving prednisone and his feverresolved after 1 week and his condition was monitored[patient outcome not clearly stated].
