Reactions 1468, p31-32 - 7 Sep 2013 S Peginterferon-α-2a/ribavirin Pulmonary sarcoidosis: case report A 43-year-old man developed pulmonary sarcoidosis during antiviral treatment with peginterferon-α-2a and ribavirin [routes not stated] for chronic hepatitis C infection. The man began receiving peginterferon-α-2a 180 µg/week and ribavirin 1000 mg/day, in 2009. In the fifth month of treatment, he reported fever, chills, sweating, chest pain, fatigue and headache. He had a fever (38°C), submandibular adenopathy, and sonorous and subcrepitant rales in his right lung. He had severe thrombocytopenia but no radiological pulmonary changes. Antivirals were withdrawn for 7 days and the man received antibacterials. Radiology after 7 days of antibacterial therapy showed a focus of right infrahilar condensation. His respiratory symptoms faded within 14 days and his thrombocytopenia improved. Radiology at 21 days showed a decrease in size of the inflammatory focus. Over the remaining 7 months of antiviral therapy, his platelet and leukocyte counts fluctuated. He received symptomatic treatment for intermittent fever, dry cough and fatigue. After 48 weeks of antiviral therapy, he again reported cough with purulent expectoration and progressive dyspnoea. Radiology showed bilateral hilar lymphadenopathy and tests showed significant pulmonary and cardiac changes. Phenotypic analysis of a bronchoalveolar lavage sample showed findings consistent with sarcoidosis. He began receiving prednisone and his fever resolved after 1 week and his condition was monitored [patient outcome not clearly stated].
