Reactions 859 - 7 Jul 2001 S Interferon-α-2a Retinopathy in a patient with type 1 diabetes mellitus and hypertension: case report Irreversible retinopathy developed in a 16-year-old boy with type 1 diabetes mellitus and hypertension while receiving treatment with interferon-α-2a for hepatitis B virus infection. The boy had been receiving interferon-α-2a 6MU every second day for approximately 2 months when he presented with a 1-month history of painless visual deterioration in both eyes. His vision was 20/200 and 20/400 in his right and left eyes, respectively. An examination disclosed the presence of bilateral cotton-wool spots, extensive intraretinal microvascular abnormalities, and scattered areas of neovascularisation involving the disk, retina and other areas. A fluorescein angiogram revealed diffuse microvascular leakage and neovascularisation of the optic nerve and retina. There was also widespread capillary dropout outside the macula. Treatment with interferon-α-2a was discontinued. The boy’s cotton-wool spots and haemorrhage resolved, but the vasculopathy persisted and bilateral neovascular glaucoma developed. He underwent extensive photocoagulative and surgical interventions but, despite this, his visual acuity decreased to light perception and no light perception in the right and left eye, respectively, due to massive fibrovascular proliferation. Retinopathy in a patient with type 1 diabetesmellitus and hypertension: case report
