Reactions 1491, p21-22 - 8 Mar 2014 S Interferon-β-1b Cutaneous and pulmonary sarcoidosis: case report A 33-year-old woman developed cutaneous and pulmonary sarcoidosis while receiving interferon-β-1b [Betaseron]. The woman, who had a history of multiple sclerosis, started receiving interferon-β-1b [dosage and route not stated]. Two- and-a-half years later, she developed painful erythematous lesions on her legs consistent with erythema nodosum. One week later, she developed pain, swelling and erythema of her left breast, chills, dry cough and fever. Upon breast examination, a tender mass measuring 7cm × 7cm was found. Chest CT scan found unilateral hilar adenopathy, and breast sonography revealed an abscess with peripheral fat inflammation. Following drainage, histopathologic examination revealed multiple granulomas composed of histiocytes and infiltration with giant cells, suggesting granulomatous mastitis. Based on the findings of hilar adenopathy, the granuloma in biopsy, and erythema nodosum, she was diagnosed with sarcoidosis. Interferon-β-1b was withdrawn. The woman started receiving glatiramer acetate, and she was treated with prednisolone and indometacin. She improved in 4 weeks.
