Reactions 1469, p27 - 14 Sep 2013 S Interferon-β/ribavirin Septic shock causing disseminated intravascular coagulation: case report A woman in her 60s [age not stated] developed septic shock, which subsequently caused disseminated intravascular coagulation (DIC), while receiving interferon-β and ribavirin [routes not stated]. The woman was admitted to hospital where she started receiving IV drip infusion interferon-β 6 000 000 IU/day and ribavirin 600 mg/day (morning 200mg and evening 400mg) [route not stated] for chronic hepatitis C. On day 1 of her treatment, she developed pyrexia and she started receiving loxoprofen and rebamipide. She developed inappetence on day 2 of interferon-β and ribavirin, and she started receiving omeprazole and mosapride on day 6. IV drip infusion of interferon-β 6 000 000IU was started after dinner on day 6; the patient was subsequently noted to have significant vomiting, BP of 86/40mm Hg, HR 114 beats/min and a temperature of 41.4°C. Oedema was affecting her intestinal tract wall and gallbladder wall and she was experiencing watery stool. The woman started receiving dopamine to maintain her BP and was admitted to an ICU where she was intubated and mechanically ventilated. She was diagnosed with septic shock and interferon-β and ribavirin were discontinued on day 6. Laboratory investigations showed that her platelet count was reduced; this was attributed to DIC caused by sepsis. Despite treatment, she did not recover and died of multi-organ failure 3 days after interferon-β and ribavirin had been discontinued. Pharmaceuticals and Medical Devices Agency.
