ONCE-DAILY SIMPLIFY TREATMENT FOR HYPERTENSIVES A daily dose of nadolol may have the edge on propranolol in lowering systolic BP After an initial wash-out period of 5 weeks, 75 hypertensive patients (mean supine diastolic BP , 18mm Hg) received either nadolal 40mg once daily or propranolol 1 Omg qid for 12 weeks in a double-blind, randomil .ed. comparative study. Doses were increased stepwise unt il supine diastolic BP was reduced to 90mm Hg or lower. Final daily doses ranged from SO-320mg; nadol ol 240 mg was most effective, while propranolol doses varied widely. The supine diastolic BP was normalized ( .... 90mm Hg) in 33 of 45 patients given nadol o] (7 J %) and in 17 of the 30 patients given pro pranolol (5 7 96 ). The average decrease in supine systoli c BP was 43mm Hg ( 22 .2% drop) for nadolal and 32 mm Hg (16.8 % drop) for propranolol (p <"0.0 1 ), indicating a greater effect of nadolol on th is parameter. Heart rates decreas ed by about 25 %' with each drug. The only adverse reactions thaI occurred in this series were slight weight gain in 2 patients treated with nadolol and moderate dizziness in I patient lreated with propranolol. EI Mehairy. The greatest fall in BP occurred dur ing the first 4 weeks of drug treatment , with li ttle change after tha t. Increases in heart rate and systolic BP af ter exerci se teslS were similar at 3 and 23 hours, although in all cases the values for metoprolol were slightly lower than Ihose for alenolol. The increase in systoli c BP at 23 hours was twice that observed at 3 hours.
