Reactions 1324 - 23 Oct 2010 S Interferon-α-2a/interferon-α-2b Anterior ischaemic optic neuropathy: 2 case reports Two women developed anterior ischaemic optic neuropathy during treatment with interferon-α-2a or interferon-α-2b [routes not stated] for hepatitis C. A 54-year-old woman was diagnosed with chronic hepatitis C. She received ribavirin and interferon-α-2a [Roferon-A] 9 million IU/week. She became negative for HCV RNA in the third month, and was scheduled to complete 1 year of treatment. In the sixth month of treatment, she developed blurred vision in her right eye. Examinations revealed optic disc oedema, peripapillary splinter haemorrhages and altitudinal defect in her right eye. A visual evoked potential (VEP) test in her right eye indicated optical nerve damage characterised by delayed response with low amplitude. A fundus fluorescein angiogram of her right eye revealed leakage in the late phase. Interferon-α-2a was discontinued and ribavirin was continued. In the following 6 months, her retinal haemorrhages and optic disc oedema decreased; however, she developed optic atrophy. A 57-year-old woman was diagnosed with hepatitis C. She received ribavirin and interferon-α-2b [Intron-A] 1.5 µg/kg/week. In the ninth month of treatment, she developed blurred vision in her right eye. Optic disc oedema and peripapillary splinter haemorrhages were observed. A visual field test identified altitudinal defects in both eyes, with her right eye more severe. VEP test indicated an axonal and demyelinising-type condition at prechiasmatic level in her right eye. A fundus fluorescein angiogram revealed right eye leakage during the late phase. Examinations revealed iron deficiency anaemia and primary hyperthyroidism. Ribavirin and interferon-α-2b were discontinued. After 4 months, her optic disc oedema and retinal haemorrhages had decreased; however, she developed optic atrophy. Her last best corrected visual acuity was 15/100 in her right eye and 10/10 in her left eye. Argin MA, et al.
