Anterior ischaemic optic neuropathy: 2 casereports
Two women developed anterior ischaemic opticneuropathy during treatment with interferon-α-2a orinterferon-α-2b [routes not stated] for hepatitis C.
A 54-year-old woman was diagnosed with chronichepatitis C. She received ribavirin and interferon-α-2a[Roferon-A] 9 million IU/week. She became negative forHCV RNA in the third month, and was scheduled tocomplete 1 year of treatment. In the sixth month oftreatment, she developed blurred vision in her right eye.Examinations revealed optic disc oedema, peripapillarysplinter haemorrhages and altitudinal defect in her righteye. A visual evoked potential (VEP) test in her right eyeindicated optical nerve damage characterised by delayedresponse with low amplitude. A fundus fluoresceinangiogram of her right eye revealed leakage in the latephase. Interferon-α-2a was discontinued and ribavirin wascontinued. In the following 6 months, her retinalhaemorrhages and optic disc oedema decreased; however,she developed optic atrophy.
A 57-year-old woman was diagnosed with hepatitis C.She received ribavirin and interferon-α-2b [Intron-A]1.5 µg/kg/week. In the ninth month of treatment, shedeveloped blurred vision in her right eye. Optic discoedema and peripapillary splinter haemorrhages wereobserved. A visual field test identified altitudinal defects inboth eyes, with her right eye more severe. VEP testindicated an axonal and demyelinising-type condition atprechiasmatic level in her right eye. A fundus fluoresceinangiogram revealed right eye leakage during the late phase.Examinations revealed iron deficiency anaemia and primaryhyperthyroidism. Ribavirin and interferon-α-2b werediscontinued. After 4 months, her optic disc oedema andretinal haemorrhages had decreased; however, shedeveloped optic atrophy.
