Mineralocorticoid and tlucoeortlcoJd excess after long terlll fntranasal use: first report A 28-year-old man was hospitalised for hYpertension and hyperkalaemia. He had a Long history of rhinitis for which. !te h.ad been tak.ing 2-3 containers/week of 9nfluoroprednisolone (1 Omgl container) for a 20-month period. During this time he had gained II kilos in weight, his BP had risen, and depression and anxiety developed, Four months before admission he had passed a calcium-phosphate stone, Hospital examination showed external manifestations of Cushing's syndrome, and clinical, bioch.emical and haematological evidence of glucocorticoid excess. When 9ufluoroprednisolone was stopped, aU abnormalities progressively disappeared: potassium chloride and spironolactone normalised serum potassium within 18 days; c10nidine reduced BP to normal within 30 days; moon face and truncal obesity Quickly improved.
