Reactions 1021 - 2 Oct 2004 Peginterferon-α2a/ribavirin IgA nephropathy: case report A 54-year-old man developed IgA nephropathy during treatment with peginterferon-α-2a and ribavirin for chronic hepatitis C infection. Nine days after starting treatment with SC peginterferon- α-2a 180 µg/week and ribavirin 600mg twice daily, the man presented with macroscopic haematuria. Urinalysis revealed > 1000 × 10 6 /L RBCs and > 1000 × 10 6 /L WBCs, and a 24 hour urine collection revealed proteinuria of 1.78 g/day. His serum creatinine level had increased from 0.09 mmol/L at baseline to 0.19 mmol/L, his haemoglobin level had decreased from 172 to 140 g/L and he had a bilirubin level of 44 mmol/L, a lactate dehydrogenase level of 741 U/L and a haptoglobin level of < 0.06 g/L. A kidney biopsy revealed acute tubular necrosis, glomerular IgA deposition and mild mesangial proliferation, consistent with IgA nephropathy. Peginterferon-α-2a and ribavirin were discontinued and the man’s symptoms gradually resolved. Gordon A, et al.
